Once it arrived at our workshop, the cause became clear. The previous owner had been running straight water, and over time it had corroded the thermostat housing. The corrosion was so bad that the hose fitting had actually broken the aluminium.
We replaced the corroded parts, fitted everything properly, filled the system with the correct coolant, and pressure-tested it to make sure there were no more hidden issues.
Truck back on the road. Coolant system sealed. One very relieved customer.
